When to use this form
When a veteran is rated 100% Permanent & Total and dependents need healthcare coverage. CHAMPVA covers most medical expenses.
Required
- ✓Veteran's 100% P&T rating letter
Proves the veteran qualifies for CHAMPVA benefits.
- ✓Marriage certificate
For enrolling a spouse.
- ✓Birth certificates
For enrolling children.
Recommended (if applicable)
- +Medicare card (if applicable)
CHAMPVA can be secondary to Medicare. Include your Medicare information if you have it.
- +Other health insurance information
CHAMPVA is a secondary payer to most other insurance.
You'll need to provide
- ✓Veteran's 100% P&T status verification
- ✓Dependent information (spouse, children)
- ✓Other health insurance information (Medicare, employer)
- ✓Supporting documents (marriage/birth certificates)
- ✕Not knowing this benefit exists — many 100% P&T veterans miss it
- ✕Not reporting other health insurance (CHAMPVA is a secondary payer)
- ✕Assuming TRICARE covers dependents (it doesn't for most disabled veterans)
- ✕Not applying for all eligible dependents
